Starting school year 2024-25, we require all students to turn in a new physical every year. The physical should be done before August 2nd so that the student will be cleared to participate in sports all school year. The Concordia Prep Annual Physical Exam Form has been updated and all the other forms are in On Campus and the Concordia Prep website Under "Student Life", and "Health and Wellness". The physician completes the one-page physical form.

On Monday, July 1, a Medical Form will be available through OnCampus for parents and guardians to update their student's medical records for the year. When you log into OnCampus you will see a yellow bar at the top that says you have a form to review. Click on that link and the link for the form entitled: "2024-2025 Medical Forms and Student Information". 

On this form, you can provide information about conditions, including allergies, medical history, and physician information. You may also upload the medical forms, including the physical. This will enable the nurse's office to clear students for athletic participation. The Medical Form in OnCampus is HIPAA compliant – only the Nurse's Office has access to your student's medical records. 

 

The parent should complete the history part of the physical in On Campus. The Concordia Prep Physical Exam form, Over the Counter Medication form, Medication Administration form, and Asthma and Allergy Action Plan (if your student has asthma or allergies) are completed by the physician and also signed by the parent. The Immunization form can be obtained from your student's physician. All forms should be uploaded to your student's medical record in OnCampus. It is best to have all the forms prepared, uploaded, and completed in one sitting. The forms are also available to download here:
